Support at Home Partner – it is a Partnership!
With CS#1’s unique partnership approach, you are engaged as much as you choose to manage your support services at home. With a dedicated Support at Home Partner, you are given all the support you need to ensure your services are in place and the peace of mind that a case manager is supporting you.
Our unique service model offers you a dedicated Support at Home Partner, who will provide regular contact, support, guidance and monitoring of your supports at home. CS#1 also has Registered Nurses that can provide clinical expertise when required.
LOCAL PROVIDER WITH LOCAL KNOWLEDGE AND PERSONAL APPROACH
When you join CS#1 you’ll get
1. A dedicated Support at Home Partner.
Your Support at Home Partner will take time to understand your needs, preferences and goals.
Regular check-ins will be agreed with you.
2. Access to CS#1 support workers, or your choice of any other providers support worker.
We will give you genuine choice on the support workers you feel comfortable with.
3. Clinical oversight from a Registered Nurse
Your health and wellbeing is a priority to CS#1 which is why we will dedicate the access to a Registered Nurse.
