Home Care Package

If you require higher levels of support or have more complex needs, a Home Care Package may suit you. It is personalised ongoing support with a fixed amount of money provided by the Australian Government to help you keep your independence and lifestyle while continuing to live in your own home.

There are four levels of Home Care Packages, ranging from basic through to high level care needs, and which are provided based on an assessment of a person’s needs.

  • Level 1 meets basic care needs
  • Level 2 meets low-level care needs
  • Level 3 meets intermediate care needs
  • Level 4 meets high-level care needs

Home Care Packages are provided on a Consumer Directed Care (CDC) basis. If you are eligible for a Home Care Package, you can decide who you want as your service provider. If you select us (Community Services #1), we will meet with you to set your goals and determine to best assistance to meet your needs.

We specialise in providing expert care advice, care and package management, budgeting, transport and social support whilst arranging high quality cleaning, personal care, gardening and other supports through our own dedicated team of home care workers and  specialised service providers. This approach means that you have more choice and control over your care. This approach means we can offer lower prices for popular services such as cleaning and nursing care.

Community Services #1 is a profit-for-purpose and has been providing services to the Canberra community since the late 1980’s. We currently serve over 500 seniors through various different State and Federal Government programs. Our experienced team have been providing services to seniors in the local community for many years. Eligibility

You may be eligible for services under the Commonwealth Home Support Programme or a Home Care Package if you are:

  • 65 years or older, or 50 years or older and identify as an Aboriginal and Torres Strait Islander person, or 50 years or older and on a low income, homeless or at risk of homelessness
  • still living at home
  • in need of help at home to continue to live independently.

Assessment for entry level home support

To find out if you need an assessment and if you are eligible for help at home services, contact My Aged Care on 1800 200 422 or visit www.myagedcare.gov.au. The My Aged Care staff will ask you questions about your current needs and circumstances so they can refer you to appropriate aged care services, such as Community Services #1.

You will need to have a Regional Assessment Service assessment before you can be approved for care. Find out more about a RAS assessment.

After your assessment outcome

Once your assessment is complete, you will receive a letter of approval from the government’s My Aged Care that sets out the level of Home Care package you’re approved for. Community Services #1 can then work with you to create a package that suits your needs.

What are the amounts for the Government subsidy and how is it determined?

The Government subsidy is allocated according to care needs and is broken down into levels 1 – 4 (4 being the highest). The ACAT (Aged Care Assessment Team) assesses each client and determines their level of funding according to their level of needs.
