If you require support at home, you may be eligible for a Home Care Package supported by the Commonwealth government. CS#1 can assist you to understand the process to apply for a Home Care Package and provide you with the personalised supports you may require.
CS#1 works with you to ensure your Home Care Package is personalised to your support and needs. If you are eligibility the government will provide you the ability to source services from an approved Home Care Provider, like CS#1 to assist you to maintain your independence and safety in your own home.
You may be eligible for one of the four levels of Home Care Packages, ranging in levels of supports required which are provided based on an assessment completed by the government on your personal needs. The four levels of annual eligible subsidy are:
- Level 1 $10,271 to meet basic support needs
- Level 2 $18,063 to meet low-level support needs
- Level 3 $39,310 to meet intermediate support needs
- Level 4 $59,593 to meet higher level support needs
CS#1 provides a person-centred approach with Home Care Package case management, that is designed for you and with you, ensuring your supports are tailored to your individual needs. Our care management services are designed to meet your needs, you will be assigned a dedicated Support at Home Partner, who will be your key contact in developing your support plan, individualised budget. Between CS#1’s case and financial management we will assist you to meet to set your goals and partner with you to ensure your supports are being meet.
CS#1 specialises in providing expert support advice, case and package management with Home Care Packages. Our Home Care Package services ensure you have a updated support plan, individualised budget and regular scheduled phone and face to face check ins with a dedicated Support at Home Partner.
CS#1 can offer direct services in the areas of domestic cleaning, transport, social support and personal care. Sourcing CS#1 direct support workers will enable you to save and to have more funds in your package for additional hours of support. If you choose to engage with an alternate service, CS#1 can accommodate this for you providing you with choice and control on services of your choosing. CS#1 will work with you to ensure all your services i.e. gardening, physio or one-off items which can be supported by the Home Care Package program are engaged and provided to assist you maintain your quality of life. Additionally, we will provide you with monthly statements outlining the services you have received against your eligible Home Care Package subsidy from the government.

Eligibility
You may be eligible for services under the Commonwealth Home Support Programme or a Home Care Package if you are:
- 65 years or older, or 50 years or older and identify as an Aboriginal and Torres Strait Islander person, or 50 years or older and on a low income, homeless or at risk of homelessness
- still living at home
- in need of help at home to continue to live independently.
Assessment for entry level home support
To find out if you need an assessment and if you are eligible for help at home services, contact My Aged Care on 1800 200 422 or visit www.myagedcare.gov.au. The My Aged Care staff will ask you questions about your current needs and circumstances so they can refer you to appropriate aged care services, such as Community Services #1.

After your assessment outcome
Once your assessment is complete, you will receive a letter of approval from the government’s My Aged Care that sets out the level of Home Care package you’re approved for. Community Services #1 can then work with you to create a package that suits your needs.

What are the amounts for the Government subsidy and how is it determined?
The Government subsidy is allocated according to care needs and is broken down into levels 1 – 4 (4 being the highest). The ACAT (Aged Care Assessment Team) assesses each client and determines their level of funding according to their level of needs.
