About the role and CS#1 employee rewards:

The Support at Home Partner position, facilitates and delivers Home Care support and services tailored to individual client service needs, under a person-centred approach that will enable them to stay at home safely and independent as much as possible.

The position will contribute to the delivery of CS#1’s Support at Home services through the delivery of high qualify aged care programs.

  • Full-time position ongoing – Support at Home Partner
  • Hourly rate range – $41.52-$44.68, commensurate with qualifications and experience, plus 11% superannuation and salary packaging
  • Social, Community, Home Care and Disability Services Award Level 4

What you will be responsible for as a Support at Home Partner: 

  • Deliver care management services in accordance with the Aged Care Quality Standards, Aged Care Act and Home Care Programme Operational Manual
  • Escalate any risks associate with clients and seek assistance to address and mitigate any immediate or ongoing concerns.
  • Implement, monitor, and review support plans and budgets to meet individual client preferences, needs and goals. Escalate Request for Reviews with My Aged Care as required for eligibility for the Home Care Support Program.
  • Ensuring key performance indicators are met including timely collection and distribution of data for client statements and invoicing.

Support at Home Partner requirements:

To apply for this position, you must have experience in either the Commonwealth Home Support and/or Home Care Packages Programs and a sound understanding of monitoring budgets, service delivery networks and resources available to support older people in their homes.

  • Demonstrated equivalent Certificate IV qualifications in relevant fields such as Community Services, Health Services, Aged Care, or equivalent work experience.
  • Registered Nursing experience (highly desirable) from a home and community care setting.
  • Demonstrated knowledge and experience of Home Care Packages or Commonwealth Home Support Program and supporting legislation, aged care quality standards and reform agendas.
  • Demonstrated experience and well-developed skills in the delivery of telephone and face-to-face assessment and referral services to individuals, families, and communities with diverse and complex cultural, emotional, and social needs.
  • Demonstrated sound understanding of risk assessment and duty of care for client services.
  • Excellent communication skills and experience working in a team environment.
  • ACT Working with Vulnerable People Card
  • National Police Check
  • Proof of COVID-19 vaccination
  • Current unrestricted driver licence and own transport
